Important Passport Information

The Old Bridge Public Library is pleased to offer Passport processing service.

Please take a few minutes to review this information before your visit to our Passport office.

  • Passport applications are accepted on a first-come, first-serve basis. We do not accept reservations. Due to the high volume of applications, there are times when we reach our capacity for the day and are unable to accept additional applications.

  • The acceptance of a passport application can be time-consuming. Plan your visit at the beginning of our posted hours. This is especially important if applying for passports for multiple family members.
  • We will do everything we can to help you track the      progress of your application; but, we have no control over its actual processing. Processing time may fluctuate according to workload in the US Passport processing offices.
  • For a child under age 16, both parents or the child’s legal guardian(s) must appear when submitting a passport application. If only one parent appears, you must also submit one of the following:
  1. Second parent’s notarized written statement         consenting to passport issuance for the child, or
  2. Primary evidence of sole authority to apply, or
  3. A written statement (made under penalty of perjury) explaining the second parent’s unavailability.

What’s Needed to Apply?

Form DS-11 if applying for the first time or under age 16.
or
Form DS-82 if you can submit a previous U.S. passport
issued to you in the last 15 years, when you were age 16 or older.
